How To Create Health ID Card

There are 2 way to Create Health ID card

  1. Viya Aadhar Card
  2. Viya Mobile Number

Health ID Card Requed Documents

There are some mandatory documents to upload with the application form of PM Modi Health ID Card 2021

  • Passport Size Photo
  • Mobile Number

Using a Health ID is the first step towards creating safer and efficient digital health records for you and your family. You can opt-in to create a digitally secure Health ID, which allows you to access and share your health data with your consent, with participating healthcare providers and payers


  • First of all, They have to open the official website that is  nha.gov.in with the help of the internet
  • From the home page of the official website Click on the “Create Health Id” option
  • Click on the “Create your Health ID Now” option
  • Now there will be two option on your computer screen Generate Via Aadhar Card & Generate Via Mobile Number
  • if you want to create through Aadhar card then select it and if you want to create through a Mobile number then select it
  • Now fill your Aadhar number or Mobile number in the box
  • An OTP number will receive on your mobile number, fill it correctly
  • PM Modi Health ID Card registration form will open there
  • fill all the mandatory details and click on the submit button to complete the process.

Create Health ID card by Mobile Number

This is options #2 for creating a unique Health ID. You can register via providing a mobile number and verifying it via OTP.

  • Step 1: On the same page, as mentioned above, Click on “Generate via Mobile”
  • Step 2: In the next page, Enter your mobile number in the space provided and click on “Submit”
  • Step 3: Then, you will be asked to enter the OTP received
  • Step 4: Now, you will be taken to the Health ID card online registration page where you can apply for your unique Health ID Card under the “National Digital Health Mission” Scheme.
  • Step 5: Enter your basic information such as Full Name, Date of Birth, Password, and Gender

Note: In HEALTHID field enter your name or mobile number and note down this health id because it useful for account recovery.

How to Create Health Id via Aadhar

Applicants who want to create their own Health id can follow this procedure

  • First of all, Applicants have to open the official website that is nha.gov.in
  • Now click on the Create Health ID option
  • A new page will open on your computer screen 
  • Then click on the “create your health id now” option
  • Click on the generate via Aadhar Card option 
  • Enter your Aadhar Card number and fill the captcha code 
  • Finally, click on the submit button
